Awarded to a junior, senior or graduate student in the College of Nursing, Health, and Sciences who:
- Majoring in Health, Physical Education and Recreation
- GPA 3.0 or higher
-Demonstrate exemplary citizenship and character
-Exhibit an appreciation for extra-curricular activities
-Show promise for future achievement in the field of Health, Physical Education, and Recreation
- Full-time attendance

Applicant must submit the following:
